The radio buttons were not responding to each other in groups, but rather to each other throughout the entire document. Field : Title, first and last name (added at the end if applicable). The signature field was set up in such a way that a placed signature would appear behind it. Corrected. Column 3, "Permissible?" , has been slightly streamlined. An information box was placed before the overview:
